Patient Paperwork Instruction:

To print the paperwork prior to your first visit, click on the Patient Forms tab, wait for the drop down tab and select which form you want to print.  A pop-up window will appear, and a pdf document will open.  Press the print icon, and your paperwork will print out.

Please print and fill out all forms as completely and accurately as possible so that we can provide the best care possible.  The more we know about your health, the better we care for you, and the better your results will be. 

If you are an adult, please print out the Adult Patient Form.  If you are filling out a form for a child under the age of 18, please fill out the Child Patient Form.  Then print, read, and sign the Consent Form.

The Toxicity Questionnaire and the System Survey will help us determine your nutritional deficiencies (if any) which may be causing a number of your health complaints.

The Outcomes Assesment Questionnaire will help us monitor your progress.  On your first visit, you will want both pages filled out. Thereafter, you will fill out page 2 every 12 visits or so.
